Complete screener to assess eligibility and provide consent (no parent or guardian consent needed)
If ineligible or no consent is provided, exit survey.

If eligible and consent is provided:
Complete Survey #1
20 Minutes | $40 E-gift card

Use website!
- Participants will be provided a login and password to access website
- Text or email reminders will be sent
- Use at your own pace for four continuous weeks
- Earn points for completing lessons which may include 2-8 activities each
- Points earned can be redeemed for e-gift cards up to $125
